Your first visit

Dear patients,

We look forward to welcoming you to our dental practice. To ensure a smooth process, we ask you to observe the following points:

• Please arrive on time for your appointment. If you are unable to attend or will be late, please inform us in good time.
• If you have statutory health insurance, please bring your insurance card with you.
• Before your first visit, please complete the medical history form, which you can download from our website or obtain from the practice. The medical history form contains important questions about your health and your use of medication that are relevant to your dental treatment.
• Please remember your bonus booklet if you have one. The bonus booklet documents your regular check-ups and can secure you a subsidy from your health insurance company for certain services.
